Thanks to twitter user @YonkouProd, a scan of the recent issue of V-Jump magazine revealed an official drawing of “Perfected” Ultra Instinct Goku, drawn by Akira Toriyama (creator of the Dragon Ball series). Along side of the manga-drawn of the new form of Goku, an anime version was showcased as well.

The new form has not “officially” aired or seen in the manga… yet. With Dragon Ball Super coming to a close end of March, expect to see this form appear. In addition to thew new form revealed in the latest V-Jump magazine, “Perfected” Ultra Instinct Goku will also be available as DLC for Dragon Ball Xenoverse 2.
